REGISTRATION / LIABILITY INFORMATION
BO KNOWS HOCKEY LTD. accepts registrations by mail, fax and e-mail. Registration will be confirmed in writing prior to the school start date. Detailed schedules will be sent prior to the program start date. Please note: BO KNOWS HOCKEY LTD. does NOT agree to hold spots for parents or players upon verbal communication.
PAYMENT: Please enclose FULL payment. You may pay the total owing with two cheques: half payable at the time of mailing your application, the remaining post-dated no later than three weeks prior to program start date. Players will not be permitted on the ice the first day without payment in full. Your cancelled cheque(s) is your receipt.
CANCELLATION AND REFUND POLICY: All cancellations must be made in writing Full refunds will be honored for MEDICAL REASONS only. A copy of the doctor's certificate will be required. |

WAIVER OF LIABILITY - PLEASE READ CAREFULLY
We the applicant and his/her parents agree that BO KNOWS HOCKEY LTD. It's owners, directors, and instructors along with the ice arena, recreational facility and its employees are not liable for nor held responsible for any incident or loss however caused and agree to release same from all claims and damages. We further agree that the applicant has no medical problems and is in good physical health and we will be responsible for all medical, dental and/or insurance claim. BO KNOWS HOCKEY LTD. reserves the right to use any photos taken during our programs for advertising and/or instructional purposes. BO KNOWS HOCKEY LTD. reserves the right to cancel, regroup or reschedule players based on ability and enrollment.
